Mais dans l’ombre de ses triomphes il doit faire face à des ennuis de santé. Victime de décollements de la rétine et il est opéré en novembre 1989. On lui greffe des implants. Cette situation sonne généralement la fin de carrière pour un boxeur. Pas pour Jackson, poussé qui plus est par son manager Carl King, qui doit faire vivre sa famille (sa femme Debra lui a donné cinq enfants) et reconstruire sa maison détruite par l’ouragan Hugo. Après avoir essuyé des refus, le combat contre Herol Graham, titre WBC des moyens en jeu, est accueilli en Espagne. L’Anglais domine les trois premiers rounds. Il a fermé l’œil gauche de Jackson qui frappe dans le vide. Il dira plus tard avoir vu «trois Graham sur le ring». L’arbitre Joe Cortez menace d’arrêter, mais au milieu du 4e round, la droite de Jackson foudroie le Britannique. Ce KO reste comme l’un des plus impressionnants de l’histoire. Alors que Graham est encore inconscient, Jackson tombe à genoux pour remercier dieu. Le myope conserve le titre pendant deux ans et demi en fauchant Dennis Milton (1er), Ismaël Negron (1er) et Ron Collins (5e), alors que Thomas Tate lui impose d’attendre le verdicts des juges, chose qu’il n’avait plus vécu depuis plus de dix ans…

A chacune de ses prestations la tension est palpable. Pourtant Jackson, sans doute trop effacé en dehors du ring, ne sera jamais au sommet de l’affiche lors des grandes soirées de Las Vegas. C’est toujours le cas, le 8 mai 193, lorsqu’il trouve sur sa route Gerald McClellan, un autre terrifiant puncheur. Alors qu’il est en tête sur les cartes des juges après quatre rounds, Jackson s’effondre sur une terrible droite au menton. La revanche, un an plus tard, est encore plus frustrante pour Jackson, KO après… 83 secondes de combat. S’il redevient champion WBC des moyens le 17 mars 1995, face à l’Italien Agostino Cardamone (KO 2e), le cogneur des Iles Vierges tombe de son trône cinq mois plus tard, face à Quincy Thomas (KO 6e). Cette quatrième défaite annonce la fin de carrière du Faucon, qui connaît deux derniers échecs avant de raccrocher avec un palmarès de 55 victoires, dont 49 avant la limite, et 6 défaites. «Mon rêve est de revenir sur le ring pour un championnat dans le coin de mes fils.» Julius (mi-lourds) et John (welter) ont participé aux JO de Pékin, mais n’ont pas hérité de son punch…
